For the first seven days you are off sick you do not need a doctor’s note. You may need to ‘self certify’ by getting a form from your employer.
If you have been in hospital and require a sick note you may phone for a note. Please give your name, particulars of your illness and how long you expect to be off, to the receptionist. The form will normally be ready within 48 hours. Sick notes can be backdated if necessary.
If you are unable to carry out your job and require extended periods of time off work you will need to be reviewed by your doctor regularly.
