The usage of the Community Hospitals is currently the subject of a review process which aims to ensure that the hospitals support both the Acute Trust modernisation agenda and the needs of local populations via their primary and social care services.
Intermediate care is delivered in each of the Community Hospitals. The service offers time limited rehabilitation with the aim of preventing avoidable admissions to hospital and facilitating early discharge. Some intermediate care is also delivered within the day hospital setting, the aim being to develop intermediate care services within the community.
